I authorize the Sioux Falls School District (SFSD) to make any investigation of any personal, educational, vocational, or employment history. I further authorize any current or former employer, person, firm, corporation, educational, or vocational institution or government agency to provide the SFSD with information they have regarding me. I hereby release and discharge the SFSD and those who provide information from any and all liability as a result of furnishing and receiving this information. I further agree that falsification of any part of this application, including any accompanying materials, shall be sufficient cause for dismissal. References and personal information which become a part of this application will be regarded as confidential and shall not be revealed to me. 

I understand that this application is not a contract or offer of employment.  I also understand that if hired, regardless of any oral representation to the contrary, the employment between myself and the SFSD is terminable-at-will so that both the Sioux Falls School District and I remain free to choose to end our work relationship at anytime for any reason or no reason, subject to collective bargaining agreement terms.

Further, I understand the SFSD is drug free/tobacco/tobacco-replacement/vapor product free.  All employees are subject to SFSD’s Code of Conduct policy.
